Dear Parent/Guardian:

In partnership with Impactus | Promise Keepers Canada, we are joining an event that requires your permission for your son to participate. We have provided you the details of the event and request that you complete and sign the permission form. Please note that all activities have risks. Your minor (Minor is defined as being older than 12 years of age and under the provincial legal age)’s safety is our primary concern. Precautions will be taken for your wellbeing and protection.

Impactus | Promise Keepers Canada sponsors live events designed for men 20 years of age and older.

Impactus | Promise Keepers Canada call men to live Christ centered, God honouring lives, loving and prioritizing their faith and family while addressing the outside demands on their time and resources. The attendees of our events come from surrounding areas and are on different stages of their faith exploration and experience. We often discuss common issues that men struggle with including, purity, pornography, greed, lust, addictions, pride, anger, etc.

Impactus | Promise Keepers Canada provides security on site and recommend that individuals and host churches join us in taking precautions for safety and the protection of participants. However unforeseeable issues could arise including accidents, slips and falls, injuries, violence or threats of violence, assaults and outrages, abuse, leaving individuals injured or harmed.

While every precaution is taken for the safety and good health, events carry with them the inherent risk of personal injury beyond the risks associated with many recreational activities. I understand that I am exposing myself to inherent risks and hazards. I accept all these risks and hazards and agree that I will be responsible for any injury or other loss which may occur during my participation in these events.

I, undertake and agree to indemnify and hold blameless Impactus | Promise Keepers Canada and the host church, its Personnel, its leaders and Board from and against any loss, damage or injury suffered by the myself as a result of being part of the events of Impactus | Promise Keepers Canada. This consent and authorization are effective only when attending a Impactus | Promise Keepers Canada event.
